On or about October 19, 2012, the ground was littered with scrap lumber that had protruding nails, exposing employees to a puncture hazard.

29 CFR 1926.1052(c)(1)(i):  Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail:    a.  On or about October 18, 2012, inside the house, the stairway leading to the second floor did not have at least one hand rail, exposing employees to a fall hazard.

On or about October 18, 2012, inside the house, the stairway landing did not have a guardrail, exposing employees to a 10 feet fall hazard.

P & S Service Group, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.102(a)(1) which was contained in OSHA inspection number 315484188, citation 01, item number 003(a), issued on March 9, 2012, with respect to a workplace at 4525 Plantation Oaks, Middleburg, Florida 32068. The final order date was March 20, 2012.    P & S Service Group, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.102(a)(1) which was contained in OSHA inspection number 313875411, citation 01, item number 002, issued on March 23, 2010, with respect to a workplace at 15300 Spotted Stallion Trail, Jacksonville, Florida 32234. The final order date was April 20, 2010.      29 CFR 1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a.  On or about October 19, 2012, on the 6:12 pitch roof, employees performing framing were not protected from a 24 feet hazard by the use of a fall protection system.
