29 CFR 1910.212(a)(3)(ii):Point of operation guard were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a. 3255 Shawnee Industrial Way, Suwanee, GA (Rosenthal Machine Area) - Employees were exposed to struck-by hazard. Employees are operating and/or unjamming the Rosenthal Machine which is housed with a blade (point of operation) to cut rubber and paper material to size. The Rosenthal Machine was not adequately guarded at the point of operation. Employees can lift up the plexi-glass (guard) while machine is operating. There was no interlock switch and/or light curtain to stop the machine from operating once the plexi-glass is moved, on or about 10/23/12.

29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control: a. 3255 Shawnee Industrial Way, Suwanee, GA (Rosenthal Machine) - Employees were exposed to sudden energization or start-up of multi-energy source equipment. The company's lockout/tagout machine specific procedures did not include specific requirements for testing the machines or equipment to determine and verify the effectiveness of lockout devices, tagout devices and other energy control measures. Equipment includes but is not limited to, Rosenthal Sheeter Machine, Car Mat Line, Press 5 Cells, Press 6, Chromomjet A, Chromomjet B, and Chromomjet C, on or about 10/23/12.
