OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to burn hazards: a)The Millennium Mat Company - The employer did not ensure that the extruder head on the car mat line was covered or isolated to prevent employees from making contact with parts of 180 degrees F to 286 degrees F while working and walking adjacent to the extruder head on or about October 23, 2012. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

(b)Press # 5 and #6-mold transfer area-numerous chains and sprockets approximately 3 1/2 feet from the floor.

Under section 17(c) of the Occupational Safety and Health Act of 1970 and 29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: (a)Production area-The employer did not perform periodic inspections of the energy control procedures to assure that the procedures were being followed and corrections were made as needed. Equipment include, but is not limited to,rosenthal sheeter machine, car mat line, press 5, press 6, chromom Jet A, chromom Jet B, and chromom Jet C, on or about 10/23/12. THE MILLENNIUM MAT COMPANY, LLC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1910.147(c)(6)(i) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314202722, CITATION NUMBER 02, ITEM NUMBER 1, ISSUED ON OCTOBER 7, 2010. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
