29 CFR 1926.501(b)(11):  Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by a guardrail systems with toeboards, safety net systems, or personal fall arrest systems.    Instance A:    This violation occurred on November 14, 2012, at a workplace located at 8108 R.L. Thornton Fwy., Dallas, Texas 75228; where at least eleven (11) employees were observed working on a commercial roof without the use of fall protection.    Instance B:    This violation occurred on November 30, 2012, at a job site located at 8108 R.L. Thornton Fwy., Dallas, Texas; where at least one (1) employee was observed working on a commercial roof without the use of fall protection.
