29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) On or about 11/19/12, in Area C of the Holley High School Roof, 3800 N. Main St., Holley, NY: Employees were exposed to a fall of approx. 10 foot through any of the seven skylights in the roofing work area. No fall protection system was utilized. ABATEMENT CERTIFICATION REQUIRED
