29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a. Building 5 third floor, a red and a yellow extension cords had the ground prong broken off. The cords were not provided with a grounding conductor thus exposing employees to electrical hazards. b. Building 5 third floor, a portable band saw had the grounding prong broken off. The saw was not provided with a grounding conductor.
