29 CFR 1926.501(b)(12): Each employee engaged in the erection of precast concrete members (including, but not limited to the erection of wall panels, columns, beams, and floor and roof "tees") and related operations such as grouting of precast concrete members, who is 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:    a.  On or about February 16, 2017, at the sixth level of the garage building, two employees installing and securing precast double-T beams were not protected from a 62 feet fall hazard from the edge and 11 feet fall hazards from the inside by the use of a fall protection system.    b.  On or about February 16, 2017, at the fifth level of the garage building, one employee grabbing welding leads and doing welding work was not protected from a 51 feet fall hazard from the edge by the use of a fall protection system.

29 CFR 1926.502(g)(1)(ii): When erecting precast concrete members, the control lines were not erected at least 6 (1.8m) nor more than 60 feet (18m) or half the length of the member being erected, whichever is less, from the leading edge:    a.  On or about February 16, 2017, at the fifth level of the garage building, the warning line erected by the employer was less than 6 feet from the leading edge, exposing employees to a 51 feet fall hazard.

On or about February 16, 2017, at the north fifth level of the building, one employee climbed on a closed 8-foot Werner stepladder against the wall to do work.        b. On or about February 16, 2017, at the north fifth level of the building, one employee climbed on a closed 6-foot Werner stepladder against the wall to do welding work.          c. On or about February 16, 2017, at the south fifth level of the building, one employee climbed on a closed 8-foot Werner stepladder against the concrete beam to do work.
