29 CFR 1910.147(c)(7)(i):     The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:    Cardington Yutaka Technologies, Inc., located at 575 West Main St., Cardington, OH 43315: On or about October 30, 2012, the employer failed to properly train employees to lockout/tagout Cat/Con Line 10 Water Leak Test # 1 (manufactured by Matumoto Sangyo Co., Ltd., Serial # 04S047) before employees conducted activities such as, but not limited to, unjamming or cleaning the machine. Employees were exposed to the unexpected startup of the machines moving parts while conducting work, and that could result in struck-by and caught-in injuries.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.147(d)(2):     The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment:    Cardington Yutaka Technologies, Inc., located at 575 West Main St., Cardington, OH 43315: On or about October 30, 2012, the employer failed to implement established energy control application steps to control hazardous energy of the Cat/Con Line 10 Water Leak Test # 1 (manufactured by Matumoto Sangyo Co., Ltd., Serial # 04S047) during a time employees accessed hazard areas of the machine. Employees were exposed to struck-by and caught-in injuries because the machines energy sources were not shut down and controlled prior to unjamming or cleaning the equipment.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.303(g)(1)(i)(A):     For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening where they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910:    Cardington Yutaka Technologies, Inc., located at 575 West Main St., Cardington, OH 43315: On or about October 30, 2012, the employer failed to ensure a clearance of at least 3 feet in front of Cat/Con Line 10 Cover Assembly # 1 electrical cabinet (480V, 3-Phase; Number 160-330072) that is located adjacent to the Line 10 Cover Assembly # 1 cell. The electrical cabinet was blocked by a yellow cart that was used for the placement of defective parts.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
