On or about 12/07/12, at the site of 29-20 Healy Ave. Far Rockaway, NY Employees working at the site were in danger of head injury from falling objects such as, but not limited to tools and roof materials from the roof. Protective helmets were not provided. ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used: A). On or about 12/07/12, scaffolds at 29-20 Healy Ave. Far Rockaway, NY Employees were ascending/descending a scaffold by climbing the frame and were in danger of falling approximately 24 ft to the ground. An access ladder or equivalent safe devices were not provided. ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.
