29 CFR 1926.451(c)(3):     Supported scaffold poles, legs, posts, frames, and uprights shall be plumb and braced to prevent swaying and displacement.      A)  132 Carmita Avenue, Rutherford - NJ:  Employees were exposed to fall hazards/scaffold collapse hazards of approximately 24 feet while working on a pump jack scaffold that was not braced at the and was installed on a pitched roof, on or about October 18th 2012.

A)  132 Carmita Avenue, Rutherford - NJ:  Employees were exposed to fall hazards of approximately 24 feet while working on a pump jack scaffold that was not protected by a complete guardrail system or other means of fall protection in place, on or about October 18th 2012.

29 CFR 1926.501(b)(13):     Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):       A)  W.Central Avenue, Palisades Park - NJ: Employees were exposed to fall hazards of approximately 14 feet whle engaged in siding work on a residential structure. The employees were not protected by any type of fall protection system, on or about October 18th 2013.

29 CFR 1926.1053(b)(1):     Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet  above the upper landing surface to which the ladder was used to gain access:      A) 132 Carmita Avenue, Rutherford - NJ: Employees were exposed to fall hazards of approximately 16 feet while using an extension ladder to access a roof. The extension ladder that was used, did not extend at least 3 feet above the landing, on or about October 18th 2012.
