29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary.    a) Exterior right wall: Employee was exposed to a fall hazard of approximately 18 feet while engaged in vinyl siding activities from a pump jack scaffold that was not braced on the bottom, on or about 10/15/14.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   a) Exterior right wall: Employees without any head protection were engaged in siding activities bellow other employee involved in the same activities being exposed to head injury, on or about 10/15/2014.  Cajamarca Construction Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 311922306, citation number 01, item number 02 and was affirmed as a final order on 3/3/2010, with respect to a workplace located at 109 Churchill Ave. Somerset NJ.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.   a) Exterior right wall: Employee was exposed to a fall hazard of approximately 18 feet while engaged in vinyl siding activities from a pump jack scaffold that did not have a complete guardrail system or other means of fall protection in place, on or about 10/15/14.    Cajamarca Construction Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 789561, citation number 01, item number 02 and was affirmed as a final order on 5/24/13, with respect to a workplace located at 132 Carmita Ave., East Rutherford NJ.  Cajamarca Construction Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451 (g)(4)(i), which was contained in OSHA inspection number 311922306, citation number 2, item number 01 and was affirmed as a final order on 3/3/2010, with respect to a workplace located at 109 Churchill Ave. Somerset NJ.
