29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe: a) At 7119 Rocky Road Loop, Gulf Shores, Alabama: On or about December 14, 2012 and at times prior; the employer exposed his employees to amputation hazards in that he allowed them to use a Makita circular saw when the guard had a piece of wood wedged in it to keep the guard in the up position exposing the blade. ) At 7119 Rocky Road Loop, Gulf Shores, Alabama: On or about December 14, 2012 and at times prior; the employer exposed his employees to amputation hazards in that he allowed them to use a Makita circular saw that did not have a guard covering the bottom portion of the cutting blade.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) At 7119 Rocky Road Loop Gulf Shores, Alabama: On or about December 14, 2012 and at times prior; the employer exposed his employees to fall hazards of 11.8 feet to the ground below in that he did not provide fall protection for employees installing plywood on a roof. b) At 7119 Rocky Road Loop Gulf Shores, Alabama: On or about December 14, 2012 and at times prior; the employer exposed his employees to fall hazards of 19.6 feet to the concrete floor below in that he did not provide fall protection for employees climbing the rafters to get on a roof. c) At 7119 Rocky Road Loop Gulf Shores, Alabama: On or about December 14, 2012 and at times prior; the employer exposed his employees to fall hazards of 9.2 feet to the concrete floor below in that he did not provide fall protection for employees installing plywood on the floor of the attic. d) At 7119 Rocky Road Loop Gulf Shores, Alabama: On or about December 14, 2012 and at times prior; the employer exposed his employees to fall hazards of 9.5 feet to the ground below in that he did not provide fall protection for employees involved in framing activities from the roof over a bay window.
