North Star Terminal, Maintenance Shop:  On or about 18 December 2012 and times prior thereto, the employer did not conduct a personal protective equipment assessment for chain saw operations.  Chain saws are used to cut up dunnage. Chaps and face protection was not provided nor worn. This condition exposed employees to being struck by flying debris, cuts and amputations.

A random sampling of chemicals found in the workplace was taken; the following products were selected and did not have a Safety Data Sheet readily available: FPPF ABC Air Brake Conditioner, Mark V products window cleaner; System One Parts Washers, Sherwin Williams Xylene and Krylon Rust Tough. This condition exposed employees to chemical burns and skin dermatitis.
