29 CFR 1910.305(b)(1)(ii):     Unused openings in boxes, cabinets, or fittings were not effectively closed.    (a) North Star Terminal, Maintenance Shop:  On or about 18 December 2012 and times prior thereto, the 120 volt circuit breaker panel box located on the east wall by the shop bay door and the circuit breaker panel located in the main bay area were missing blanks.   This condition exposed employees to a 120 volt electrical shock hazard.    (b) North Star Terminal, Maintenance Shop:  On or about 18 December 2012 and times prior thereto, the 120 volt electrical outlet located on the east wall by the shop bay door was missing its cover.   This condition exposed employees to a 120 volt electrical shock hazard.    (c) North Star Terminal, Maintenance Shop, Main bay area:  On or about 18 December 2012 and times prior thereto, a 120 volt junction box missing a knock out.  This condition exposed employees to a 120 volt electrical shock hazard.

29 CFR 1910.305(g)(2)(iii):     Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws.    North Star Terminal, Maintenance Shop, Main bay area:  On or about 18 December 2012 and times prior thereto, the plug on an electrical cord was pulled from its insulation, exposing electrical wires on the tire changing machine.  This condition exposed employees to a 120 volt electrical shock hazard.

29 CFR 1917.112(a)(1):     Vehicle curbs, bull rails, or other effective barriers at least 6 inches in height were not provided at the waterside edges of aprons and bulkheads.    North Star Terminal, Dock area:  On or about 18 December 2012 and times prior thereto, there was no vehicle curb, bull rail or effective barrier installed on the dock area where an employee was pushing snow with a front end loader into the Cook Inlet.  This exposed employees to the hazard of falling into the water.

29 CFR 1917.112(b)(1):     Guardrails were not provided at locations where employees were exposed to floor or wall openings or waterside edges, including bridges or gangway-like structures leading to pilings or vessel mooring or berthing installations, which presented a hazard of falling more than 4 feet or into the water.    Main shop area:  On or about 18 November 2012 and times prior thereto, the stationary ladder to mezzanine did not have a railing installed in the cargo opening or a ladderway installed on the ladder to keep employees from falling.  The ladder measured approximately 12 foot in height to mezzanine deck.  This condition exposed employees to a fall hazard.

29 CFR 1917.119(e)(1):     The employer did not ensure that ladders with defects listed under 1917.119(e)(1)(i)-(v) were not used and either were tagged as unusable or removed from the worksite.    North Star Terminal, Maintenance Shop, west bay area:  On or about 18 December 2012 and times prior thereto, the Werner 8 foot ladder being used by the electrician in repairing lighting in the east bay had a damaged lower brace.  This condition exposed employees to a fall hazard.

29 CFR 1917.154:     Employees shall be protected by chip guarding and personal protective equipment complying with the provisions of Subpart E of this part during cleaning with compressed air.  Compressed air used for cleaning shall not exceed a pressure of 30 psi. Compressed air shall not be used to clean employees.    North Star Terminal, Maintenance Shop, Main bay area:  On or about 18 December 2012 and times prior thereto, two (2) an air nozzles used for cleaning registered approximately 100 psi.  Air nozzle was not equipped with a deflector to reduce the pressure of air flow.  This exposed employees to air embolism / laceration hazards if they contacted their skin.

North Star Terminal, Maintenance Shop, Main bay area:  On or about 18 December 2012 and times prior thereto, the mezzanine did not have the max safe load limits identified on the structure.  This condition exposed employees to a fall in collapse hazard.
