OSH ACT of 1970 Section (5)(a)(1):     The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees    Yard area: On or about 19 December 2012 and times prior thereto, employees are using a fall arrest system (shock absorbing lanyard) when tethered to an aerial lift. This condition exposed employees to a fall hazard.     Among other methods, one feasible and acceptable method to correct this hazard includes but not limited to the following: Replace the fall arrest device with an approved fall restraint device.

29 CFR 1910.305(b)(1)(ii):     Unused openings in boxes, cabinets, or fittings were not effectively closed.    (a) Tool can: on or about 19 December 2012 and times prior thereto, the employer did not ensure that used openings in electrical boxes and panels were effectively closed. Two electrical boxes had knockouts removed. The electrical breaker panel was missing a knockout on top of the panel. This condition exposed employees to a 120 volt electrical shock hazard.    (b) Shop bay area, compressor room: electrical panel had three unused open blanks that were not effectively closed. Knockout was missing on the left side of the electrical panel. This condition exposed employees to a 120 volt electrical shock hazard.

29 CFR 1910.303(g)(2)(i):     Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i).    Shop bay area, compressor room: An overhead heater had 120 volt live electrical wires that were not guarded from accidental contact. This condition exposed employee to an electrical shock and electrical burn hazards.

29 CFR 1910.304(f)(1)(iv):     Overcurrent devices for circuits rated 600 volts, nominal, or less, were not readily accessible to each employee or authorized building management personnel.    Shop bay, compressor room: On or about 19 December 2012 and times prior thereto, the electrical panel room was not readily accessible to each employee. Five gallon containers of oil were stored next to panel. This condition exposed employees to a placement hazard in the event that electrical power needs to be removed.

Shop bay area: On or about 19 December 2012 and times prior thereto, employer did not ensure that dunnage, lumber, and shoring material had protruding nails removed from the immediate work area. In the shop bay area, a wooden pallet had nails approximately two inches in length, protruding upward. This condition exposed employees to a puncture hazard.

Shop bay: On or about 19 December 2012 and times prior thereto, walking and working surfaces in the shop bay area was not maintained in good repair. A floor opening measuring 14" wide, 108" long, with a depth of 6" was partially filled with wood scraps with nails and pieces of metal. This condition exposed employees to puncture hazards.
