29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, shall be kept cleared from work areas, passageways, and stairs, in and around buildings or other structures. (a) Construction Site - On or about December 19, 2012 employees were exposed to multiple fall and puncture hazards while working. Employees were exposed to falling on boards and boards with nail protruding from them. Six instances with boards with nails where obsered in different locations.

29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents. (a) Front scaffold - On or about December 19, 2012 an employee was exposed to being struck by flying objects while using a nail gun. Employee was not wearing safety glasses. (b) Front of House - On or about December 19, 2012 employee was exposed to being struck by flying objects while using a circular saw. Employee was not wearing safety glasses.

Employees were not protected from falling to lower level by a personal fall arrest system or a guardrail system.
