29 CFR  1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:   a) Exterior of Building - Front: Employee without any means of fall protection was standing on a plywood/pallet platform being elevated by an All Terrain Lull Front End Loader being exposed to fall hazards of approximately 8-10 feet, while engaged in framing activities, on or about 6/10/2021.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Exterior of Building - Front: An employee engaged in Framing Activities was exposed to fall hazards of approximately 13ft  while using a portable aluminum extension ladder whose side rails were not raised 3ft above the upper landing surface.
