29 CFR 1910.252(a)(2)(xiii)(D): The employer did not recognize its responsibility for the safe usage of cutting or welding equipment, and did not advise all contractors about flammable materials or hazardous conditions of which they may not be aware:    (a) Facility, Creek Building, Bridgeville, PA:  On or about December 19, 2012 the employer did not recognize its responsibility for the safe usage of cutting or welding equipment on its property and did not advise all contactors about flammable materials or hazardous conditions.  The employer had not conducted an inspection of the workplace to access what combustibles, flammables or other potential hazards existed in the area prior to permitting cutting and welding by an outside contractor.  The employer did not ensure the contractor understood what precautions needed to be taken when working in the area such as providing a fire watch or covering potential hazards in the work area.  A fire occurred at the plate leveler from sparks and hot materials from the cutting operations and the contractor did not have a fire watch in effect nor was the area covered with fire blankets.
