29 CFR 1910.107(b)(5)(i):  The average air velocity over the open face of the spray booth was less than 100 linear feet per minute.    a) Spray Room #2: The employer failed to ensure that the air velocity over the surface of the spray room was sufficient to move flammable vapors through the exhaust duct and maintained an air velocity of at least 100 linear feet per minute. Employees were exposed to the hazards of a fire and/or explosion while utilizing methyl ethyl ketone and other flammable materials to spray paint components for the manufacture of agricultural equiprment. Velometer readings ranged 75-90 linear feet per minute and indicated that a restriction existed in the air velocity.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s)     a)  Production Area: The employer failed to ensure that a Daewoo (model # G30P) and a Toyota (model # 8FG432) industrial truck were removed from service when the horn and/or other warning signals were not functioning. Employees were engaged in operation of the forklift trucks daily to move goods in and around the production area for the manufacture of agricultural equipment.     b) Production Area: The employer failed to ensure that industrial forklifts (Daewoo; Model #G30P and Toyota; Model # 8FG432) maintained seat belts in a condition that was safe for operation. Employees were utilizing forklifts that had broken seat belts and were exposed to being crush while moving heavy parts in and around the production area.
