29 CFR 1926.100(a):     Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.    (a)  Base of Right Scaffold - On or about February 4, 2013 an employee was exposed to head injuries while tossing bricks to another employee on the second level of a scaffold 10 feet 4 inches above the ground. Employees were not provided hardhats.

29 CFR 1926.452(c)(2):     Frames and panels shall be braced by cross, horizontal, or diagonal braces, or combination thereof, which secure vertical members together laterally. The cross braces shall be of such length as will automatically square and align vertical members so that the erected scaffold is always plumb, level, and square. All brace connections shall be secured.    (a) Left scaffold - On or about February 4, 2013 employees were exposed to a fall hazard while working on a scaffold that had no cross braces installed on the inside face.

(a) Right Scaffold - On or about February 4, 2013 employees were exposed to a fall hazard while climbing on a welded metal frame scaffold. The scaffold ladder was not aligned where it could be used for egress or access.

29 CFR 1926.451(g)(1):     Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.  Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part.    (a)  Right Scaffold - On or about February 4, 2013 employees were exposed to a fall hazard while working on a welded metal frame scaffold 10 feet 4 inches above ground with no guardrails installed.    (b)  Left Scaffold - On or about February 4, 2013 employees were exposed to a fall hazard while working on a welded metal frame scaffold 10 feet 4 inches above ground with no guardrails installed.

29 CFR 1926.454(a):     The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable:    (a)  Construction Site - On or about February 4, 2013 employees were exposed to a fall hazard while working on scaffolds without training on how to recognize, minimize and prevent fall hazards.
