29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    a) Continental Structural Plastics, Inc., North Baltimore, Ohio:  On or about February 5, 2013, the employer did not ensure that employees who operated the #3 Hepburn manufactured (Serial 203043) 1,500 Ton hydraulic power press were protected from crushing and amputation injuries. The point of operation was not adequately guarded, in that the light curtains were not adjusted correctly to protect employees who were loading and unloading materials from the press bed.  Additionally, only one set of two hand controls were being used when there were three employees attending to the press.    Add instance b) to the violation description: per iSA  	  	b)  Continental Structural Plastics, Inc., North Baltimore, Ohio:  On or about February 5, 2013, the employer did not ensure that employees who operated the #3 Hepburn manufactured (Serial 203043) 1,500 Ton hydraulic power press, were protected from crushing and amputation injuries.  The alignment cylinders were not guarded to prevent employees from inadvertently accessing this area during a press cycle.        Continental Structural Plastics, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA Inspection Number 311607360, citation number 1,  item number 2 and was affirmed as a final order on June 30, 2009 with respect to a workplace located at 2915 CR 96, Carey, Ohio.
