29 CFR 1926.1051(a):  Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:      a.  On or about February 11, 2013, employees did not have safe access and egress from the second floor to the first floor, exposing them to an 8 feet fall hazard.

Chris Sawdo Construction, LLC was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection 313882425, Serious citation number 01, 002, issued on March 23, 2011, with respect to a workplace at 6089 Little Colt Court, Jacksonville, FL, with a Final Order date of May 3, 2011.    29 CFR 1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a.  On or about February 11, 2013, on the 5:12 pitch roof, employees installing OSB roofing material were not protected from a 10 feet fall hazard by the use of a fall protection system.
