29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  a. Employees installing sheathing at elevations of six feet or more above the ground are not being protected from fall hazards by a fall protection system:  The employer fails to assure effective protection against falls are required by the standard, as demonstrated on April 13, 2016, when employees installing sheathing on a 6:12 pitch roof of a house located at 478 Heron Landing Road, in St. Johns Florida were exposed to a 20 feet 7 inch fall hazard above lower level in that they were not protected by a fall protection system.  Chris Sawdo Construction, LLC was previously cited for a Willful violation with 11B language of this occupational safety and health standard, which was contained in OSHA inspection number 978000, citation number 2, item number 1 and was affirmed as a final order on October 22, 2014, with respect to a workplace located at 153 South Twin Maple Road in Saint Augustine Florida.  Chris Sawdo Construction, LLC was previously cited for a Repeat violation of this occupational safety and health standard, which was contained in OSHA inspection number 879783 citation number 2, item number 1 and was affirmed as a final order on March 5, 2013, with respect to a workplace located at 4668Karsten Creek Drive Jacksonville Florida.  Chris Sawdo Construction, LLC was previously cited for a Willful violation of this occupational safety and health standard, which was contained in OSHA inspection number 315479832, citation number 1, item number 1 and was affirmed as a final order on January 23, 2012, with respect to a workplace located at 11096 Lothmore Road Jacksonville Florida.
