29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:    a.   On or about May 23, 2014, on the back side of the house, employees descended from the top cords of trusses and the roof by climbing the interior framing of the structure, exposing themselves to a 14 feet fall hazard to a concrete slab.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:    A.   On or about May 23, 2014, the roof access ladder used by employees did not extend at least 36 inches above the edge of the eaves, exposing employees to a 9 feet fall hazard.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects were not immediately marked "Defective, Do not Use" and withdrawn from service:    a.   On or about May 23, 2014, the aluminum extension ladder used by employees for roof access ladder that had dented and cracked rungs, dented and cracked rails, and corroded and loose rung lock bolts was not withdrawn from service.  Employees accessing the roof with the damaged ladder were exposed to a maximum 9 feet fall hazard from ladder collapse.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:    a.   Employees working from elevation of 8 to 18 feet while installing roof sheathing are not being protected from the hazard of falling from these elevations to the concrete slabs.  The employer has not installed fall protection systems, nor required the use of personal fall protection systems.  This violation was most recently observed on or about May 23, 2014, at a job site located at 153 Twin Maple Rd, Saint Augustine Florida.  On the roof sheathing and top cords of trusses, employees installing roof sheathing were exposed to an 8 to 18 feet fall hazard in that they were not protected by a fall protection system.    To abate this violation, the employer must ensure that all employees working at elevations of 6 feet or greater are protected from falls by a fall protection system.      Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance, including and explanation of how these steps protect its employees from fall hazards.
