The boxes contained a 110 VAC electrical outlet that supplied power to 220 VAC portable power tools.

29 CFR 1926.403(i)(2)(i): Except as required or permitted elsewhere in this subpart, live parts of electric equipment operating at 50 volts or more was not guarded against accidental contact by cabinets or other forms of enclosures, or by any of the following means: (a) Inside the building near the rear east wall on or about February 13, 2013, there was no cover on a 220 VAC panel box. The box was 13 inches wide by 31.5 inches high, and the bottom was 58 inches from the floor. The box supplied power to portable power tools.

29 CFR 1926.405(a)(2)(ii)(B): All conductors were not protected by overcurrent devices at their ampacity. Runs of open conductors were not located where the conductors will not be subject to physical damage, and the conductors were not fastened at intervals not exceeding 10 feet (3.05 m): (a) Inside the building in an electric panel box near the rear east wall on or about February 13, 2013, number 14 wire, inside non-metallic sheathed cable, was attached to 40 amp, 220 VAC circuit breakers. The wire size must be at least number 10. (b) Inside the building near an electric panel box near the rear east wall on or about February 13, 2013, non-metallic sheathed cable, lay on the floor. The cable supplied power to an outlet in a metal box on the floor.

29 CFR 1926.405(b)(2): All pull boxes, junction boxes, and fittings were not provided with covers. When metal covers are used, they were not grounded. In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass were not provided with bushings designed for the purpose or they did not have smooth, well-rounded surfaces on which the cords may bear: (a) Inside the building near an electric panel box near the rear east wall on or about February 13, 2013, there was no cover plate on the 110 VAC outlet located in a handy box that lay on the floor. The box contained a 110 VAC electrical outlet that supplied power to 220 VAC portable power tools.
