29 CFR 1917.112(a)(1): Vehicle curbs, bull rails, or other effective barriers at least 6 inches in height were not provided at the waterside edges of aprons and bulkheads. (a) 1 Port Island: On or about February 15, 2013, and times prior thereto, there was no barrier installed along the terminal access road and the waterside edge. This exposed vehicles and/or employees to falls down the edge into the water.

29 CFR 1917.26(b): A first aid kit and at least one person holding a valid first aid certificate were not at the terminal when work was in progress. (a) 1 Port Island terminal: on or about February 15, 2013, and times prior thereto, the employee on-site did not have a valid first aid card. This exposed employees to a delay in getting proper medical attention in the event of an accident.
