29 CFR 1917.48(a)(1): Danger zones at or adjacent to conveyors were not guarded to protect employees: The Dalles West Terminal, Transfer Tunnel On or about April 22, 2015 and at times prior thereto, two (2) 7 and 1/2 inch flanged shafts of the Transfer Drag Motor/Reducer located 20 inches above the platform were not guarded. This condition exposed employees to caught-in hazards.

29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: The Dalles West Terminal: On or about April 22, 2015 and at times prior thereto, fit-testing was conducted where employees had facial hair that came between the sealing surface of the facepiece and the face. This condition impacted the fit of the respirator, affected the facepiece seal, and exposed employees to inhalation hazards.
