29 CFR 1917.112(b)(1): Guardrails were not provided at locations where employees were exposed to floor or wall openings or waterside edges, including bridges or gangway-like structures leading to pilings or vessel mooring or berthing installations, which presented a hazard of falling more than 4 feet or into the water. (a) Platform Dust Bin: On or about April 22, 2015 and times prior thereto the platform had no guardrails. This exposed employees to a 10 foot fall hazard to a lower level. (b) Multiple platforms ladder-way to top of bin : On or about April 22, 2015 and times prior thereto there was no guardrail or swing gate at ladder access points. This exposed employees to over a 25 foot fall hazard. (c) Upper platform top of ship loading conveyor: On or about April 22, 2015 and times prior thereto there was no guardrail or swing gate at the top of the ladder-way. This exposed employees to a 25 foot fall hazard.

29 CFR 1917.118(d)(6): The side rails of ladders did not extend at least 36 inches above the top landing surface. Pit area: On or about April 22, 2015 and times prior thereto, the ladder coming out of the pit to not extend above the floor or did not have a grab-bar. This exposed employees to a 10 foot fall back into the pit lower landing.
