(a)  Platform Dust Bin:  On or about April 22, 2015 and times prior thereto the platform had no guardrails.  This exposed employees to a 10 foot fall hazard to a lower level.  (b)  Multiple platforms ladder-way to top of bin :  On or about April 22, 2015 and times prior thereto there was no guardrail or swing gate at ladder access points.  This exposed employees to over a 25 foot fall hazard.  (c)  Upper platform top of  ship loading conveyor:  On or about April 22, 2015 and times prior thereto there was no guardrail or swing gate at the top of the ladder-way. This exposed employees to a 25 foot fall hazard.
