29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person:    (a)  Urbina Construction, Inc. at 2791 W. 52nd Ave., Denver, CO 80221: On and before 2/22/13, the employer did not ensure that the supported scaffold was designed by a qualified person, in that the scaffold wood platform was constructed at the job site by non-qualified personnel.  A JLG TL943 rough terrain fork-lift was used to elevate an employee approximately 12 feet on a platform in order to perform work.  This condition exposed the employee to a fall hazard of approximately 12 feet.

This condition exposed the employee to a fall hazard of approximately 12 feet.
