29 CFR 1926.404(b)(1)(i): The employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors:    (a) On the deck in front of the project on or about February 21, 2013, there was no assured equipment grounding conductor program or ground-fault circuit-interrupters to protect employees that used a 110 VAC Hilti drill, powered through an extension cord, to install decorative rope rails on the perimeter of the deck.
