29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact.  Construction site, permit #B14000763 - Employees were observed not wearing protective eyewear at the work site; one employee had been cutting wood with a Makita circular saw and another was rolling latex interior paint overhead.

29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites.  Construction site, permit #B14000763 - Ground fault circuit interrupter protection or an assured equipment grounding conductor program was not used when a 110 VAC Makita circular saw was powered through an extension cord.  Employees were exposed to the hazards of contacting electricity.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement:    Construction site, built-in pool, permit #B14000763 - About 24 number 4 rebar protruding from 24"- 36" above ground were not protected.  Employees that placed form work and walk in this area are exposed to impalement hazards.
