29 CFR 1910.1450(e)(1): Where hazardous chemicals as defined by this standard are used in the workplace, the employer shall develop and carry out the provisions of a written Chemical Hygiene Plan: On or about February 25, 2013, in the facility lab, employees (Laboratory Technicians) required to perform sample testing/analysis were exposed to the hazard of contact with hazardous chemicals including, but not limited to, Phenol and Alpha-Metyl Styrene (AMS). The employer did not develop and carry out the provisions of a written Chemical Hygiene Plan.

29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment: On or about February 25, 2013, in the facility lab, employees (Laboratory Technicians) required to perform sample testing/analysis were exposed to the hazard of contact with hazardous chemicals including, but not limited to, Phenol and Alpha-Metyl Styrene (AMS).
