29 CFR 1910.119(e)(3)(i): The process hazard analysis shall address the hazards of the process; Cumene Unit - On or about December 13, 2013, a vapor release occurred on the Propane Receiver overhead pipe line to Feed Surge Drum that was blinded. The 2 inch diameter blinded pipe line cracked/split leaving the line open to atmosphere. The Hazard and Operability Study (HAZOP) conducted on July 30, 2009 did not address the deviations/hazards of the process. Employees were thereby exposed to the hazards associated with flash fires.
