29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used: a) roof - employees were exposed to a fall of 21 feet 3/4 inches while installing tar paper. The employees were not trained on how to erect the fall protection system so that it provides fall protection 100 percent of the time, on or about 2/25/13.
