29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) Employees installing roofing materials at elevations of 6 feet or more above the ground are not being protected from fall hazards by a fall protection system: The employer fails to assure effective protection against falls as required by the standard, as demonstrated on February 7, 2019, when three employees installing roofing materials on the 5:12 pitch roof of a house located at 18928 Willowmore Cedar Drive in Lutz, Florida, were exposed to an over 18 foot fall hazard to the ground, in that, fall protection equipment was not installed or utilized.  Brad McDonald Roofing & Construction Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1335632 , citation number 1, item number 1 and was affirmed as a final order on 2/6/2019, with respect to a workplace located at 3174 Oliver Creek Dr., Odessa, FL.  Brad McDonald Roofing & Construction Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1324557, citation number 1, item number 1 and was affirmed as a final order on 7/24/2018, with respect to a workplace located at 2948 Murray Pass, Odessa, FL. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   (a) For employees working at 18928 Willowmore Cedar Drive, Lutz, FL, as observed on or about February 7, 2019. Employees were exposed to an over 18 foot fall hazard to the ground, in that, the extension ladder did not extend 3 feet above the roof edge.   Brad McDonald Roofing and Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1207554, citation number 1, item number 1 and was affirmed as a final order on May 30, 2017, with respect to a workplace located at 3965 Aldea Way, Wesley Chapel, Florida.
