29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Employees installing roofing materials at elevations of 6 feet or more above the ground are not being protected from fall hazards by a fall protection system: The employer fails to assure effective protection against falls as required by the standard, as demonstrated on March 7, 2019, when two employees installing roofing materials on the 5:12 and 6:12 pitch roofs of a house located at 4530 Long Branch Lane in Palmetto, Florida, were exposed to an approximate 20 foot fall hazard to the ground, in that, fall protection equipment was not installed or utilized.  Brad McDonald Roofing & Construction Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1335632 , citation number 1, item number 1 and was affirmed as a final order on 2/6/2019, with respect to a workplace located at 3174 Oliver Creek Dr., Odessa, FL.  Brad McDonald Roofing & Construction Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1324557, citation number 1, item number 1 and was affirmed as a final order on 7/24/2018, with respect to a workplace located at 2948 Murray Pass, Odessa, FL. 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) For employees working at 4530 Long Branch Lane, in Palmetto, as observed on or about March 7, 2019. Employees were using pneumatic nail guns without the use of any eye protection. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) For employees working at 4530 Long Branch Lane in Palmetto, as observed on or about March 7, 2019. Employees were using a ladder that did not have the side rails extended a minimum of 3 feet above the upper landing surface. The employees were exposed to a fall height of about 10 feet.
