29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) At 308 Bentlet Court Auburn, Alabama: On or about February 25, 2013 and at times prior; the employer failed to ensure employees installing shingles on a high slope roof were using fall protection exposing them to fall hazards of 13.5 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) At 308 Bentley Court Auburn, Alabama: On or about February 25, 2013 and at times prior; the employer failed to ensure a portable ladder used by employees to gain access to and from the roof extended at least three feet above the landing area exposing employees to a fall hazard of 13.5 feet to the ground below.
