Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards:     (a) On or about April 2, 2013, and at times prior thereto, employees were exposed to fall and/or fire hazards while working on the derrick board. The auxiliary escape line did not have the carriage/buggy attached to the cable so as to facilitate a fast means of escape in the event of a fire or other emergency. Nomac Drilling Rig #323 location is Well TML 14-13 #1H near Trenton, ND.     (b) On or about April 2, 2013, and at times prior thereto, employees were exposed to fall and/or fire hazards while working on the derrick board. The auxiliary escape line was secured to a hose reel at the ground level and no one on site knew if the hose reel would hold a 3,000 pound pull weight. Nomac Drilling Rig #323 location is Well TML 14-13 #1H near Trenton, ND.     Abatement Note:  Among other methods, one feasible and acceptable method of abatement to correct this hazard includes but is not limited to following the manufacturer's installation instructions and the API RP 54-1995 Occupational Safety for Oil and Gas Drilling, section 6.10.1, to ensure that the carriage/buggy is attached to provide a ready and convenient way to escape and that the ground anchor will withstand a 3,000-pound pull.
