29 CFR 1926.50(g): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: At the project site on April 10, 2013, an employee mixing and applying Portland cement, was not provided with an eyewash unit/station. The first 10 to 15 seconds after exposure to a hazardous substance, especially a corrosive substance, are critical. Delaying treatment, even for a few seconds, may cause serious injury.

29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact. At the project site on April 10, 2013, eye protection was not worn while an employee used a claw hammer to chip concrete blocks with a claw hammer. Employee was exposed to flying particles striking the eye.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection. At the project site on April 10, 2013, employees were exposed to electrical shock hazards while using a Ridgid R7130 ½ inch drill to mix Portland cement that was plugged into an outlet without a ground fault circuit interrupter.
