29 CFR 1926.405(b)(2): In an energized installation, each outlet box did not have a cover, faceplate, or fixture canopy: Building 810 B in the Presidio Trust: On or about April 08, 2103 and at times prior thereto, employees were exposed to an electrical shock of 120 volts when using and working in proximity to multiple electrical switches and sockets that were missing faceplates, while installing a heating system.
