29 CFR 1910.304(g)(5): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective. 1. Inside of the shipping area, employees were using Unline impulse sealer and a radio that were not grounded. The metal casing on the appliances was not grounded. The ground pin was missing from the plug on the powerstrip used to energize the appliances.

29 CFR 1910.305(b)(2): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: In completed installations each outlet box shall have a cover, faceplate, or fixture canopy. 1. In the maple storage area, near the exit door, there was a knockout missing on the side of the junction box.
