29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening:  Employees were exposed to fall hazards in that ladderway floor openings were not guarded in accordance with the standard:  a) Employees using the ladder to access the catwalk by bin 4 were exposed to fall hazards of approximately 30 feet from the catwalk to the ground, in that there was not a swinging gate or offset.  b) Employees walking and working in the mill were exposed to fall hazards of approximately 8 feet from the main work floor to the boot pit floor, in that the guard rail for the ladderway opening did not have a midrail installed on one side.
