29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent: An employee working in the Pellet forming area (upper level of Feed mill) was exposed to recorded noise dosimeter sound level of 87.7 dBA during the 430 minute sampling period on September 13, 2013.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: At boiler room where water treatment is performed, employer failed to provide suitable facilities for flushing or drenching of eyes and body, exposing employees to chemical burns and eye injury as a result of coming in contact with corrosive material named Ametrol 1100 ODT BOILER WATER TREATMENT.

29 CFR 1910.1200(f)(6)(i): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information required by 29 CFR 1910.1200(f)(1)(i) through 29 CFR 1910.1200(f)(1)(v) In butler building, secondary safety containers were not labeled accordingly exposing employees to fire hazard and inhalation hazard from Gasoline.
