29 CFR 1910.28(b)(3)(ii): Each employee was not protected from tripping into or stepping into or through any hole that was less than 4 feet (1.2 m) above a lower level by covers or guardrail systems:  a) Employees were exposed to tripping and fall hazards. Employees engaged in housekeeping and grain handling activities in the feed mill behind the South Shaker by the top floor level were exposed to tripping and fall hazards into open floor hole of 3.5 by 8.5 inches. The employer did not assure covers were readily available and installed for all floor holes. b) Employees were exposed to tripping hazards. Employees engaged in housekeeping and grain handling activities in the feed mill by Bin #11 of the top floor level were exposed to tripping hazards into an open floor hole of 3 inches in the grate. The employer did not assure covers were readily available and installed for all floor holes.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

Employees engaged in tooling activities in the Work Shop of the Feed Mill were exposed to fire and electrical shock hazards in that three (3) of the 4-inch square fourplex metal electrical boxes designed for fixed installation were used to energize portable power tools such as the Jet 10" Bench Grinder and the Dewalt saw, were wired with a black flexible cord, were not listed, labeled or certified for this application, and were not securely affixed to any surface.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, outlet boxes and fittings were not provided with covers approved for the purpose; and each fitting in completed installations did not have a cover, faceplate, or fixture canopy:  a) Employees were exposed to fire and electrical shock hazards. Employees engaged in housekeeping and grain handling activities in the Feed Mill Main Mixing Floor inside the Control Room were exposed to fire and electrical shock hazards in that the control box for the fan to run the dust system had the top knob/button missing, thus exposing energized electrical wires. b) Employees were exposed to fire and electrical shock hazards. Employees engaged in housekeeping and grain handling activities in the Feed Mill Main Mixing Floor by the scale of the main mixing area were exposed to fire and electrical shock hazards in that the Drug Scale was not effectively closed, thus there was no protection from contacting the exposed energized electrical wires.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  Employees were exposed to fire and electrical shock hazards. Employees engaged in housekeeping and grain handling activities in the elevator West side tunnel by the sump pump were exposed to fire and electrical shock hazards in that the plug-in to the outlet to power the sump pump was loose at the end connection, thus exposing energized wires.
