An employee received a rash on both hands on 1-21-13 and was treated at Concentra Medical Center. B. An employee received second degree burn on left thumb on 4-24-13 and was treated at Concentra Medical Center. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
