29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:  Mold Department: Employees are not required to wear the appropriate personal protective equipment such as, but not limited to sleeves to prevent first and/or second degree burns to the employees arms while removing parts from  hot mold machines.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  Facility: Employees working with chemicals such as, but not limited molding compound and fiberglass have not been trained on the safety and health hazards.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent.    Facility: On or about May 1, 2013, the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2013.      A. An employee received a rash on both hands on 1-21-13 and was treated at Concentra Medical Center.  B. An employee received second degree burn on left thumb on 4-24-13 and was treated at Concentra Medical Center.       Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
