29 CFR 1910.303(b)(1)(ii): Electrical equipment was not free from recognized hazards, based on the mechanical strength and durability, including, for parts designed to be enclose and protected by other equipment, the adequacy of the protection was not provided:  (a) Press 14 - On or about January 6, 2015, conductors on the left side of the operator next to the mold were not protected by conduit or other enclosures.  (b) Press 14 - On or about January 6, 2015, conductors on the right side of the press next to the metal stairs were wrapped around the guardrail between the guardrail and the stairs were not protected by conduit or other enclosures.

29 CFR 1910.303(g)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by approved cabinets or other forms of approved enclosures, or other means listed under this provision:  (a) Press #14 - On or about January 6, 2015, a large junction box between the press and the hydraulic pump assembly contained a terminal strip for attaching wires and there was no cover to prevent the employee cleaning in the area from contacting live electrical parts.  Suburban Plastics Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.303(g)(2)(i), which was contained in OSHA inspection number 975626, citation number 1, item number 5 and was affirmed as a final order on August 20, 2014, with respect to a workplace located at 1000 Enterprise Drive in Grenada, MS 38901.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasions:  (a) Press 14 - On or about January 6, 2015, conductors entering a 4 x 4 junction box on the edge of the wall opening behind the drill fixture was not protected from abrasions.  (b) Press 12 - On or about January 6, 2015, conductors on the back side of the press that went from a flexible seal-tight conduit to a LB fitting were exposed to abrasions.  Suburban Plastics Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.305(b)(1)(i), which was contained in OSHA inspection number 975626, citation number 1, item number 6/a and was affirmed as a final order on August 20, 2014, with respect to a workplace located at 1000 Enterprise Drive in Grenada, MS 38901.

29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:  (a) Press 14 - On or about January 6, 2015, a junction box  was missing the cover plate on a hydraulic valve to the left of the press on the hydraulic pump unit.  (b) Press 12 - On or about January 6, 2015, the plastic shop made covers were not approved for the purpose in that they were made from a plastic type material, some were held on by one or two bolts, or some were held by wire tie straps. Some were off center and did not prevent material from entering the junction box.  (c) Press 2 - On or about January 7, 2015, a 4 x 4 junction box mounted on the right side of the press near the floor did not have a cover plate to prevent material from entering the box.  Suburban Plastics Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.305(b)(2)(i), which was contained in OSHA inspection number 975626, citation number 1, item number 6/c and was affirmed as a final order on August 20, 2014, with respect to a workplace located at 1000 Enterprise Drive in Grenada, MS 38901.
