29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a) Throughout Facility - On or about July 8, 2015, there were no specific lockout/tagout procedures written for equipment, such as but not limited to, Hydraulic molding presses and Injection molding machines for controlling energy sources, such as, hydraulic pressure, thermal, mechanical (in the form of rotational motion and caught between moving platens, and potential or stored energy.

29 CFR 1910.303(g)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures, or by any of the following means identified in sub-paragraphs (A)through (D) of this paragraph:  (a) Mold Press 12 - On or about July 13 employees were exposed to live electrical parts on a micro switch on each side of the H-26-40-02 mold when filling material and removing finished parts.  Suburban Plastics Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.303(g)(2)(i), which was contained in OSHA inspection number 975626, citation number 1, item number 5 and was affirmed as a final order on August 20, 2014, with respect to a workplace located at 1000 Enterprise Drive in Grenada, MS 38901.    Suburban Plastics Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.303(g)(2)(i), which was contained in OSHA inspection number 1018165, citation number 2, item number 1 and was affirmed as a final order on May 26, 2015, with respect to a workplace located at 1000 Enterprise Drive in Grenada, MS 38901.
