29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:  Bldg. 1086, Aircraft Intermediate Maintenance Tire Shop: On May 1, 2013, and at times prior thereto, a designated exit door which led from the Tire Shop to a hallway was locked and bolted during work hours.  The U.S. Air Force 36th Civil Engineering Squadron was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.36(d)(3), which was contained in OSHA inspection number 467640, citation number 01, item number 003 and was affirmed as a final order on March 18, 2013, with respect to a workplace located at Anderson Air Force Base, Guam 96915.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  Bldg 1023, RecceTown Warehouse: On May 1, 2013, and at times prior thereto, the employer had not received written permission from the manufacturer to use the six foot extensions or a barrel attachment to transport 55 gallon drums and awkwardly shaped objects.  The U.S. Air Force 60th Logistics Readiness Squadron was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.178(a)(4), which was contained in OSHA inspection number 189976, citation number 01, item number 003a and was affirmed as a final order on August 22, 2012, with respect to a workplace located at Travis Air Force Base, CA 94535.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:   Bldg 1023, RecceTown Warehouse: On May 1, 2013, and at times prior thereto, where six foot forklift extensions and a barrel attachment were used to transport 55 gallon drums and awkwardly shaped objects, the forklifts did not have a name plate to indicate the truck's lifting capacity.  The U.S. Air Force 60th Logistics Readiness Squadron was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.178(a)(5), which was contained in OSHA inspection number 189976, citation number 01, item number 003b and was affirmed as a final order on August 22, 2012, with respect to a workplace located at Travis Air Force Base, CA 94535.

Bldg 1086, Metals Tech Shop  B. Bldg 1023, RecceTown Warehouse Crate Area  C. Vehicle Maintenance  The U.S. Air Force Civil Engineering Support Agency was previously cited for a violation of this occupational safety and health standard 1910.212(a)(1), which was contained in OSHA inspection number 191327, citation number 01, item number 001 and was affirmed as a final order on May 22, 2012, with respect to a workplace located at Travis Air Force Base, CA 94535.  The U.S. Air Force 36th Civil Engineering Squadron was previously cited for a violation of this occupational safety and health standard 1910.212(a)(1), which was contained in OSHA inspection number 467640, citation number 01, item number 004 and was affirmed as a final order on March 18, 2013, with respect to a workplace located at Anderson Air Force Base, Guam 96915.
