29 CFR 1910.23(e)(2): Stair rails did not consist of a top rail, intermediate rail and/or posts, with the vertical height not more than 34 inches (86.36 cm) nor less than 30 inches (76.2 cm) from upper surface of top rail to surface of tread in line with face of riser at forward edge of tread:   Bldg. 2469, Linen Supply: On May 3, 2013, and at times prior thereto, the standard railing of a five riser staircase did not extend the entire length of the stairs.

29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times:  Outdoor Adventure Center, Large Bay: On May 3, 2013, and at times prior thereto, two designated exit doors had broken self-closing mechanisms that would prevent the door from adequately operating during an emergency like a fire.   The U.S. Air Force 60th Logistics Readiness Squadron was previously cited for a violation of this occupational safety and health standard29 CFR 1910.37(a)(4), which was contained in OSHA inspection number 189976, citation number 01, item number 001 and was affirmed as a final order on August 22, 2012, with respect to a workplace located at Travis Air Force Base, CA 94535.  The U.S. Air Force 60th Aerial Port Squadron was previously cited for a violation of this occupational safety and health standard29 CFR 1910.37(a)(4), which was contained in OSHA inspection number 191732, citation number 01, item number 002 and was affirmed as a final order on August 22, 2012, with respect to a workplace located at Travis Air Force Base, CA 94535.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  Auto Hobby Shop: On May 3, 2013, and at times prior thereto, the Baldor Bench Grinder (s/n F1283) had a tongue guard that measured approximately a half an inch from the abrasive wheel.  The U.S. Air Force 36th Civil Engineering Squadron was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.215(b)(9) which was contained in OSHA inspection number 461962, citation number 01, item number 001 and was affirmed as a final order on December 11, 2012, with respect to a workplace located at Anderson Air Force Base, Guam 96915.  The U.S. Air Force Civil Engineer Support Agency was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.215(b)(9) which was contained in OSHA inspection number 191327, citation number 01, item number 002 and was affirmed as a final order on May 22, 2012, with respect to a workplace located at Travis Air Force Base, CA 94535.
