Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. A.) Harbor Roofing Company, Inc., 2812 West 26th Street, Erie, PA 16506, roof, rear of the structure, on or about May 7, 2013 and times prior thereto: Employees were performing roofing activities, including, but not limited to, coping metal installation. The employee was working between the warning line system and the roof edge, and was not utilizing any other means or method of fall protection. The employee was working at edge and exposed to a 12.3 feet fall hazard to the ground below.
